Advanced Practice Provider - APP - Family Medicine

About the role

SSM Health Dean Medical Group in Waunakee, WI is recruiting an experienced, full-time Family Medicine Advanced Practitioner to join a team of 5 physicians and 1 APP. You will hold your own patient panel. Previous clinical healthcare background in primary care is strongly preferred.

Responsibilities

• Outpatient-only family medicine practice
• Monday‑Friday clinic hours, no call required
• Full‑time position with flexible FTE options (0.8‑1.0, 32‑40 hrs/week including built‑in admin time)
• Design your own patient‑visit template
• Work with a mature Epic EMR system

Qualifications

• Minimum 3 years’ experience as an Advanced Practice Provider (PA or NP)
• Primary care experience preferred; urgent care or internal medicine backgrounds also considered
• Current PA/NP licensure in Wisconsin
• Family certification required for NPs; NPs must hold APRN status
• Graduation from an accredited PA/NP program
