About the Role
Provides clinical leadership to enhance compliance with quality standards and manages service delivery by inter-professional teams. Acts as a key resource for clinical and operational guidance, ensuring high-quality patient care within the scope of practice for nurse practitioners. Works under the supervision of the Clinical Director.
Responsibilities
- Manages and delivers comprehensive advanced nursing care, including assessments, post-discharge visits, care planning, and urgent interventions.
- Collaborates with patients, families, and team members to develop and revise care plans.
- Ensures compliance with care standards, participates in team meetings, and supports quality improvement initiatives.
- Performs procedures, maintains documentation, and may provide peer reviews or act as a preceptor.
- For psychiatric roles: conducts psychosocial assessments, provides mental health services, and collaborates with psychiatrists.
Requirements
- NY State RN license and Nurse Practitioner certification (adult, family, gerontology, or psychiatric-mental health).
- Relevant experience in clinical practice.
- Must maintain current licensure, certifications, and credentials.
- Excellent clinical, communication, and team leadership skills.
Preferred Qualifications
- Bilingual skills.
- Experience in home care or management.
