About the Role

Provides clinical leadership to enhance compliance with quality standards and manages service delivery by inter-professional teams. Acts as a key resource for clinical and operational guidance, ensuring high-quality patient care within the scope of practice for nurse practitioners. Works under the supervision of the Clinical Director.

Responsibilities

  • Manages and delivers comprehensive advanced nursing care, including assessments, post-discharge visits, care planning, and urgent interventions.
  • Collaborates with patients, families, and team members to develop and revise care plans.
  • Ensures compliance with care standards, participates in team meetings, and supports quality improvement initiatives.
  • Performs procedures, maintains documentation, and may provide peer reviews or act as a preceptor.
  • For psychiatric roles: conducts psychosocial assessments, provides mental health services, and collaborates with psychiatrists.

Requirements

  • NY State RN license and Nurse Practitioner certification (adult, family, gerontology, or psychiatric-mental health).
  • Relevant experience in clinical practice.
  • Must maintain current licensure, certifications, and credentials.
  • Excellent clinical, communication, and team leadership skills.

Preferred Qualifications

  • Bilingual skills.
  • Experience in home care or management.
