Advanced Practice Clinician - Bilingual Mandarin/Cantonese

About the role

Provides clinical leadership to enhance compliance with quality and care standards, managing inter-professional teams and delivering advanced nursing care within regulatory guidelines. Collaborates with healthcare professionals to develop protocols, conduct comprehensive assessments, and create treatment plans. Ensures care aligns with Patient-Centered Medical Home standards and quality measures such as HEDIS and QARR. Acts as a key resource for operational and clinical guidance, promoting team outcomes and patient satisfaction.

Responsibilities

• Manage full scope of advanced nursing practice and evaluate patient responses
• Develop and revise clinical protocols in collaboration with physicians
• Provide comprehensive patient care, including examinations, interventions, education, and discharge planning
• Coordinate with patients, families, and healthcare teams for assessment and care planning
• Ensure compliance with care standards and quality metrics for optimal patient outcomes

Requirements

• Current NY State Registered Nurse and Nurse Practitioner licenses in adult, family, or gerontology specialties
• Certified by ANCC or equivalent, with psychiatric NPs requiring PMHNP-BC certification
• Master’s degree in Nursing from an accredited program
• Minimum two years of NP experience preferred; home care or managerial experience advantageous
• Knowledge of coding, quality measures, and bilingual skills may be required
