About the role

Provides clinical leadership to enhance compliance with quality and care standards. Manages inter-professional teams, offering clinical and operational guidance to achieve optimal outcomes. Delivers advanced nursing care per state, federal, and national standards, working under the supervision of the Clinical Director for the Nurse Practitioner Program.

Responsibilities

  • Manages full scope of advanced nursing practice, evaluating patient responses and updating care plans accordingly.
  • Develops and revises clinical protocols in collaboration with physicians, providing training as needed.
  • Delivers comprehensive nursing care, including assessments, interventions, treatments, referrals, and patient education.
  • Collaborates with patients, families, and team members for assessment and care planning, ensuring continuous care management.
  • Ensures services comply with Patient-Centered Medical Home standards and quality measures.
  • Provide comprehensive advanced nursing care within scope of practice.
  • Collaborate on protocol development and training.
  • Coordinate care with patients and team members for optimal outcomes.
  • Maintain compliance with regulatory and quality standards.

Requirements

  • Current RN license and Nurse Practitioner certification in New York State, with specialty in adult, family, or gerontology.
  • Master’s degree in Nursing from an accredited program; ANCC or AANP certification required.
  • Minimum two years of nurse practitioner experience preferred; clinical home care or managerial experience advantageous.
  • Knowledge of quality measures, coding, and reimbursement processes.
  • Bilingual skills may be required.

Qualifications

  • Psychiatric Nurse Practitioners need PMHNP-BC certification; additional credentials may be preferred.
