About the Role
Provides clinical leadership by managing inter-professional teams and delivering advanced nursing care aligned with state, federal, and national standards. Acts as a clinical authority, leading care plans, assessments, and interventions across diverse settings, including virtual and home-based care. Ensures quality outcomes through collaboration with patients, families, and care teams, while maintaining compliance with healthcare standards such as PCMH, HEDIS, and QARR. Participates in on-call coverage, performs procedures within collaborative agreements, and supports quality improvement initiatives. Mentors student Nurse Practitioners and engages in professional development.
Responsibilities
- Full scope of advanced nursing practice including patient assessments, treatment planning, and interventions.
- Leading interdisciplinary team efforts and ensuring care quality and compliance.
- Educating patients and caregivers on chronic disease management and self-care.
- Managing clinical documentation, billing, and participating in quality initiatives.
- Providing care across various environments, including home visits and telehealth.
- Supporting professional growth, peer reviews, and special projects.
Qualifications
- Licensed Registered Nurse and Nurse Practitioner in New York.
- Certification in adult, family, or gerontology health.
- Master's degree in Nursing.
- Certification from ANCC or AANP.
- Minimum two years of Nurse Practitioner experience.
- Clinical home care or managerial experience preferred.
- Bilingual skills preferred.
- Maintains necessary credentials and complies with state and agency regulations.
