Advanced Practice Clinician, Per Diem

About the Role

Provides clinical leadership and manages the delivery of advanced nursing care within a multidisciplinary team, ensuring compliance with quality standards and regulatory guidelines. Acts as a clinical authority in patient assessment, treatment planning, and care coordination across various settings, including virtual, in-home, and facility-based care. Collaborates with patients, families, and healthcare providers to develop and revise care plans, ensuring optimal health outcomes. May serve as a preceptor and contribute to clinical model development and quality committees. For psychiatric roles, conducts psychosocial assessments, provides mental health interventions, and collaborates with psychiatrists.

Responsibilities

• Lead clinical teams to ensure quality and compliance
• Deliver comprehensive, advanced nursing care across settings (virtual, in-home, facility-based)
• Collaborate with patients, families, and healthcare team members to develop and revise care plans
• Perform procedures and manage documentation and billing
• Participate in quality improvement initiatives, peer reviews, and clinical committees
• Provide education on chronic disease management and self-care
• Support team performance and care coordination efforts
• Conduct psychosocial assessments and mental health interventions (for psychiatric NPs)
• Maintain professional growth, certifications, and licensure
• Participate in interdisciplinary team meetings, on-call coverage, and peer reviews

Requirements

• Current RN license and Nurse Practitioner certification in New York
• Specialty certification (e.g., adult, family, gerontology, or psychiatric-mental health)
• Master’s degree in nursing
• Relevant clinical experience
• Proficiency in coding, documentation, and quality measures
• Bilingual skills and a valid driver’s license may be required

Psychiatric-Specific Requirements

• Psychiatric-Mental Health Nurse Practitioner certification
• Relevant psychiatric clinical experience
