About the role
Provides clinical leadership to enhance quality, compliance, and care standards. Manages service delivery of inter-professional teams, offering clinical and operational guidance. Delivers advanced nursing care aligned with regulatory and national standards, working under the supervision of the Clinical Director.
Responsibilities
- Provides full scope of advanced nursing practice for targeted populations, evaluating responses and revising care plans.
- Leads interdisciplinary rounds and manages comprehensive patient assessments, post-discharge visits, and urgent interventions.
- Collaborates with patients, families, and healthcare teams for assessment and care planning, ensuring care continuity across settings.
- Ensures compliance with patient-centered medical home standards, quality measures, and documentation protocols.
- Participates in team performance improvement, quality assurance, and peer reviews.
- Performs procedures as authorized, manages documentation and billing, and may travel for home or facility visits.
- Acts as a preceptor for student Nurse Practitioners and maintains professional licensure and certification.
- For Psychiatric NPs: conducts psychosocial assessments, provides psychiatric interventions, and collaborates with psychiatrists for treatment planning.
Qualifications
- Licensed Registered Nurse and Nurse Practitioner in New York State, with specialty certification (adult, family, gerontology, or psychiatric-mental health).
- Master's degree in Nursing from an accredited program; additional certifications such as ANCC required.
- Minimum of two years of clinical practice as a nurse practitioner, preferably in home care or management, with knowledge of coding and quality measures.
- Bilingual skills may be required based on operational needs; valid driver’s license may be needed.
Pay
$200 per visit & $77.72/hour for meetings or training.
