About the Role
Provides clinical leadership to improve quality and compliance, managing inter-professional teams for individual cases or populations. Acts as a key resource, offering clinical and operational guidance to achieve optimal team outcomes. Delivers advanced nursing care aligned with state, federal, and national standards, under the supervision of the Clinical Director.
Responsibilities
- Manage the full scope of advanced nursing practice.
- Evaluate patient responses to care and adjust plans as needed.
- Lead interdisciplinary rounds and provide comprehensive care, including post-discharge visits, assessments, and urgent interventions.
- Collaborate with patients, families, and healthcare teams for assessment, care planning, and continuous management.
- Ensure compliance with care standards and participate in team performance improvement.
- Maintain accurate documentation and billing.
- Perform procedures within collaborative agreements.
- Support quality initiatives and may serve as a preceptor.
- For Psychiatric Nurse Practitioners: Conduct psychosocial assessments, provide mental health treatment, crisis intervention, and collaborate with psychiatrists.
Requirements
- Valid nursing license and nurse practitioner certification in relevant specialty.
- Experience in clinical home care or management preferred.
- Bilingual skills may be required.
- Current certifications and licensure, with ongoing professional development maintained.
