About the Role
Provides clinical leadership to enhance compliance with quality and care standards. Manages inter-professional teams, offering advanced nursing care within regulatory and national guidelines. Acts as a clinical authority, leading care planning, assessments, and interventions across various patient settings, including in-person, virtual, and home visits.
Responsibilities
- Deliver comprehensive, patient-centered care, including post-discharge, assessments, and urgent interventions.
- Collaborate with patients, families, and healthcare teams to develop and revise care plans.
- Ensure documentation, billing, and compliance with care standards.
- Participate in quality improvement, peer review, and professional development activities.
- For psychiatric roles: conduct psychosocial assessments, provide mental health interventions, and coordinate with psychiatrists.
Requirements
- Valid RN and Nurse Practitioner licenses in New York State; certification by ANCC or equivalent.
- Education: Master’s in Nursing; psychiatric roles require additional mental health certification.
- Experience: Minimum two years as a nurse practitioner, with preferred home care or managerial background.
- Strong clinical, communication, and teamwork skills; bilingual abilities beneficial.
- For psychiatric roles: experience in mental health interventions and psychiatric care.
