About the role

This position is eligible for the Education Debt Reduction Program (EDRP), a student loan payment reimbursement program. You must meet specific eligibility requirements per VHA policy and submit your EDRP application within four months of appointment. Program Approval, award amount (up to $200,000) & eligibility period (one to five years) are determined by the VHA Education Loan Repayment Services program office after review of the EDRP application. Former EDRP participants ineligible to apply.

Whole Health System of Care

The VA Midwest Health Care Network advocates for a Whole Health System of care in each of the Medical Centers. This is an approach to healthcare that empowers and equips people to take charge of their health and well-being and live their lives to the fullest. As an employee operating in a Whole Health System of care, you will operate in a model with three core elements, seeking to create a personalized health plan for each Veteran. Duties

  • Independently manages a panel of patients with acute and chronic health conditions with an emphasis on health promotion and disease prevention using evidence-based practice.
  • Develops and implements an individualized plan to achieve optimal population health and disease management.
  • Demonstrates leadership and works collaboratively with Primary Care Team and other interdisciplinary team members.
  • Orders and interprets labs, tests, and diagnostic studies.
  • Prescribes pharmacologic and non-pharmacologic interventions.
  • Initiates referrals and consults to specialty services.
  • Provides education to patients and their families.
  • Assesses and adjusts the plan of care as needed to achieve optimal patient outcomes.
  • Exemplifies the ICARE core values of-Integrity, Commitment, Advocacy, Respect, and Excellence.
  • Providers may be required to travel to the CBOC's or provide gap coverage as determined by the supervisor.
  • Provider will be required to be a designated women's provider and complete the mandatory VHA training.

Qualifications

  • Advanced Practice Nurse (Nurse Practitioner)
  • Basic Requirements:
    • Education: A post-master's certificate or master's or doctoral degree as a Nurse Practitioner.
    • Accreditation: Approved by the appropriate accrediting agency and accredited by ACEN or CCNE.
    • Licenses: Active, current, full, and unrestricted APN license to practice as an NP in a State, Territory or Commonwealth of the United States, or in the District of Columbia.
    • Board Certification: Full and current basic board certification in one of the following: Family Nurse Practitioner (FNP), Adult Nurse Practitioner (ANP), Adult-Gerontology Primary Care Nurse Practitioner (AGPCNP), or Gerontology Nurse Practitioner (GNP).
  • Advanced Practice Nurse (Nurse Practitioner) II
  • Scope, Education and Experience, and Dimension Criteria:
    • Delivers evidence-based health care, demonstrates proficiency in managing a patient panel, population, or program.
    • Utilizes research and evidence-based practice to evaluate the patient care delivery system within a patient panel or program, and contributes to changes that enhance the quality and timeliness of veteran/patient care.
    • Works effectively as a leader within a team/unit/group and seeks interdisciplinary collaboration as appropriate to enhance patient care.
    • Acquires knowledge and skills to develop expertise in area of practice.
    • Demonstrates effective communication skills and professional behavior, promoting cooperation and teamwork. Participates in interdisciplinary teams and processes that positively impact patient outcomes.
  • Advanced Practice Nurse (Nurse Practitioner) III
  • Scope, Education and Experience, and Dimension Criteria:
    • Influences and models the delivery of evidence-based health care, demonstrating expertise in managing a patient panel, population, or program.
    • Leads and implements changes that enhance the quality and timeliness of Veteran/patient care applying research and evidence in the delivery of healthcare for patient populations.
    • Initiates and leads interdisciplinary teams informed by structured approaches and processes to improve outcomes that positively impact care for a patient panel, population, or program.
    • Acquires knowledge and skills, then disseminates acquired information within and/or outside the medical facility.
    • Initiates and leads interdisciplinary teams developing systems that positively impact outcomes at the panel, population, or program level. Demonstrates effective communication skills and professional behavior, promoting cooperation and teamwork.
