About the role

The Advanced Practice Provider (APP) is an independent health care provider who coordinates care through collaboration with the attending Physician. The PA or APN provides care in accordance with the objectives, policies, and procedures of the practice and the New Jersey State Board of Medical Examiners Physician Assistant/Nurse Practitioner Advisory Committee. The main function of this role is to provide consistent, high-quality perioperative/postoperative care for all surgery patients. The APP will function as an active member of the Cardiovascular team.

Primary responsibilities include perioperative/postoperative management for cardiac catheterization patients, hospital rounds on inpatients, emergency room consultations, outpatient diagnostics (Stress Test), and medical decision making/management of Cardiovascular patients.

Responsibilities

• Healthy Lives Hospital Readmission Program: Primary Coverage for AMI and CHF with sub-specialty expansion to all HRP domains when needed.
• Therapy Management: Prescribe and adjust guideline-directed medical therapy and monitor hemodynamic data.
• Evaluation: Conduct comprehensive assessment for Chronic Heart Failure Patients.
• Multidisciplinary Coordination: Work closely with Navigator, Cardiologist, Pulmonologist, Nephrologist, Transition of Care, and all relevant members of the health care team associated with medical management and social determinants of health.
• Patient Education and Preventive Care: Educate patients and caregivers on daily fluid, sodium, and weight monitoring; guide patients on heart-healthy diets and physical activity; prevent hospital readmissions and support safe discharges to outpatient cardiac rehabilitation.
• Perform history and physical/consult examinations in preparation for cardiovascular procedures.
• Order appropriate laboratory, radiologic, and special diagnostic tests and interpret and report to surgical and cardiovascular teams.
• See emergent consults in the ED and treat conditions within the scope of the APP’s practice and experience.
• Communicate complicated cardiovascular emergencies and prepare for cardiovascular intervention.
• Assist with inpatient daily rounds as assigned.
• Assist with and perform diagnostic testing (Stress Test).
• Thoroughly explain procedures to patients prior to testing.
• Perform appropriate interpretation of laboratory/diagnostic testing.
• Responsible for discharging patients in collaboration with the attending.
• Demonstrate strong organizational skills, effective communication, and time management.
• Perform duties independently and coordinate care with the bedside nurse, attending physician, and other consulting providers.
• Serve as primary provider in the Healthy Lives Clinic.
• Compassionately interact with patients across different areas of care.

Requirements

For Nurse Practitioners (APN/NP):

• Current APN Certification
• Current RN NJ state license
• CDS
• DEA
• Current BLS/ACLS

For Physician Assistants:

• Current Physician Assistant Certification
• CDS
• DEA
• Current BLS/ACLS

Preferred: Acute care certification.

Schedule

Monday–Friday (no nights, no weekends, no holidays).

Pay

$125,000.00 – $195,000.00 per year. The compensation offered will depend on the candidate's educational background, skills, and professional experience.
