About the role
Community Bridges is seeking a full-time Adult Service Coordinator to assist individuals and families through education and support to develop and achieve their personal visions. The role involves providing direct support, identifying natural supports, and facilitating person-centered planning.
Responsibilities
- Provides direct support to families including home visits and information/referral assistance.
- Maintains up-to-date information on community resources and engages families in person-centered planning.
- Facilitates ISP development and service coordination duties.
- Ensures compliance with applicable state regulations and maintains confidentiality.
- Assists individuals and families in identifying future support needs and maintaining waiting list profiles.
- Performs other general administrative duties including record keeping and attending staff meetings.
Requirements
- Education: Associate's Degree required.
- Experience: Two years' experience in human services, developmental services, special education, or a related field required.
- Skills: Ability to communicate clearly, access community resources, advocate, represent Community Bridges, work as part of a team, collaborate, and work effectively under pressure.
- Physical Demands: Must be able to sit or stand at a workstation and perform physical tasks such as conducting home visits.
