Responsibilities

• Provide person-centered assistance with daily living activities such as personal care, meal preparation, transportation, and medication reminders.
• Implement individualized care plans to promote independence and community engagement.
• Maintain a safe, trauma-informed environment fostering client well-being and dignity.
• Document services and communicate changes in client status.
• Collaborate with interdisciplinary teams to ensure quality care.
