About the role
The Adult BHH Care Coordinator (Case Manager) is responsible for providing comprehensive care coordination services to adult patients in a healthcare setting.
Responsibilities
- Assess patient needs and develop care plans
- Coordinate referrals and services with other healthcare providers
- Monitor patient progress and adjust care plans as necessary
- Maintain accurate and up-to-date medical records
- Communicate with patients and families to ensure they understand their health status and treatment options
- Participate in ongoing professional development activities
Requirements
- Bachelor’s degree in healthcare-related field
- Minimum 2 years of relevant experience in healthcare case management
- Strong interpersonal and communication skills
- Proficiency in Microsoft Office Suite
Qualifications
- Current CPR certification
- Knowledge of HIPAA regulations
