About the role

A Community Bridges, Inc. (CBI) Adolescent Navigator assists in addressing the ancillary concerns and needs of adolescents, providing ongoing support, needs identification, intra-agency, and external coordination with the established treatment team. The role ensures supportive services and follow-up are provided before discharging patients from CBI Behavioral Health Services. The goal is to enhance success, identify and reduce barriers for clients receiving Medication-Assisted Treatment (MAT) and Behavioral Health (BH) services. The Adolescent Navigator also provides support and ongoing patient engagement while adolescents participate in CBI Services.

Responsibilities

  • Assists in addressing ancillary concerns and needs of adolescents
  • Provides ongoing support and needs identification
  • Coordinates referrals and inclusion of family supports in treatment planning
  • Connects adolescents to community supports
  • Participates in Child and Family Teams (CFT)
  • Supports Licensed Practical Nurse (LPN) and Nurse Practitioner (NP) in administrative tasks
  • Ensures supportive services and follow-up are provided before discharging patients from CBI Behavioral Health Services
  • Enhances the likelihood of success for clients receiving MAT and BH services

Requirements

  • Highschool diploma or GED and either:
    • 18 credit hours of post-high school education in a field related to behavioral health and two years of full-time behavioral health work experience;
    • Four years of full-time behavioral health work experience.
  • Associate’s degree and at least two years of full-time behavioral health work experience;
  • Bachelor’s degree in a field other than behavioral health and at least one year of full-time behavioral health work experience;
  • Master’s degree or bachelor’s degree in a field related to behavioral health.

Qualifications

  • Minimum of six months of recovery from substance use and/or mental health disorders required.
  • One to three years of substance abuse/mental health recovery-oriented service is preferred but not required.
  • Experience working with adolescents preferred.

Skills

  • Strong communication skills
  • Ability to coordinate and manage multiple tasks
  • Knowledge of behavioral health and substance use disorders
  • Experience with adolescent populations
