About the role

The Adolescent Navigator at Community Bridges, Inc. (CBI) plays a crucial role in supporting adolescents and their families. They assist in addressing ancillary concerns, needs identification, and coordination within the treatment team. The goal is to enhance the likelihood of successful treatment and reduce barriers for clients receiving Medication-Assisted Treatment (MAT) and behavioral health services.

Responsibilities

  • Provide ongoing support and engage patients in treatment
  • Participate in Child and Family Teams (CFT)
  • Support administrative tasks for Licensed Practical Nurses (LPNs) and Nurse Practitioners (NPs)
  • Coordinate referrals and connect clients to community supports
  • Incorporate family supports in treatment planning

Requirements

  • Highschool diploma or GED with 18 credit hours of post-high school education in a behavioral health-related field, or an associate’s degree with 2 years of full-time behavioral health work experience
  • Four years of full-time behavioral health work experience with a bachelor’s degree, or a master’s degree in a behavioral health-related field
  • Minimum of six months of recovery from substance use and/or mental health disorders, with 1-3 years of recovery-oriented service experience preferred
  • Experience working with adolescents is preferred

Qualifications

  • Behavioral Health Technician (BHT) license in accordance with A.A.C. R9-10-115 and CBI Clinical Policies and Procedures
  • A current AZ Driver’s License in good standing
  • A clear 39-month Motor Vehicle Record
  • An Arizona Fingerprint Clearance card, Level 1

  • Excellent communication skills
  • Ability to build and maintain relationships with clients and families
  • Strong organizational and time management skills
  • Knowledge of evidence-based treatment models
