Job Summary

An Adolescent Navigator assists in addressing all ancillary concerns and needs of adolescents, providing ongoing support, needs identification, intra-agency, and external coordination with the established treatment team. The goal is to enhance success, identify and reduce barriers for clients receiving Medication Assisted Treatment (MAT) and Behavioral Health (BH) services.

Responsibilities

  • Ensure supportive services and follow-up are provided prior to discharging a patient/participant from CBI Behavioral Health Services.
  • Provide support and ongoing patient engagement while adolescents participate in CBI Services.
  • Participate in Child and Family Teams (CFT).
  • Provide administrative support to the Licensed Practical Nurse (LPN) and Nurse Practitioner (NP).
  • Coordinate referrals, include family supports in treatment planning, and connect/coordinate to community supports.

Requirements

  • Highschool diploma or GED and either: 18 credit hours of post-high school education in a field related to behavioral health and two years of full-time behavioral health work experience; or four years of full-time behavioral health work experience.
  • Has an associate’s degree and at least two years of full-time behavioral health work experience; or Has a bachelor’s degree in a field other than behavioral health and at least one year of full-time behavioral health work experience; or Has a master’s degree or bachelor’s degree in a field related to behavioral health.
  • Minimum of six months of recovery from substance use and/or mental health disorders required.
  • One to three years of substance abuse/mental health recovery-oriented service is preferred but not required.
  • Experience working with adolescents preferred.

Qualifications

  • Behavioral Health Technician (BHT) license in accordance with A.A.C. R9-10-115 and CBI Clinical Policies and Procedures.
  • Current AZ Driver’s License (valid and in good standing).
  • Clear 39-month Motor Vehicle Record.
  • AZ Fingerprint Clearance card, Level 1.

  • Ability to build and maintain positive relationships with clients, families, and colleagues.
  • Strong organizational and time management skills.
  • Knowledge of evidence-based models for substance use and behavioral health treatment.
