About the Role

The Navigator provides comprehensive case management, care coordination, outreach, and navigation services to transition-age youth and young adults (ages 16–26). As a trusted partner, the Youth Navigator helps participants identify strengths, overcome barriers, and access coordinated services that promote health, education, employment, housing stability, behavioral health, and overall well-being.

The role involves building collaborative relationships with community providers, educational institutions, workforce partners, healthcare organizations, and social service agencies to ensure participants receive timely, coordinated, and person-centered care. This position uses trauma-informed, culturally responsive, and youth-driven practices to empower young adults toward self-sufficiency and long-term success.

Responsibilities

  • Conduct outreach and engagement activities to identify and connect with eligible youth and young adults through schools, community organizations, healthcare providers, workforce partners, and community events.
  • Complete participant intake, screening, strengths-based assessments, and eligibility determination for COE services.
  • Develop individualized service and goal plans in collaboration with participants, incorporating education, employment, health, behavioral health, housing, transportation, and other supportive service needs.
  • Provide ongoing care coordination through regular meetings, follow-up contacts, home or community visits (as appropriate), and progress monitoring.
  • Serve as the primary point of contact for participants, providing guidance, advocacy, encouragement, and system navigation throughout their participation in the COE.
  • Coordinate referrals and facilitate access to internal and external resources, ensuring warm handoffs and continuity of care.
  • Build and maintain collaborative partnerships with community agencies, educational institutions, healthcare providers, behavioral health organizations, employers, and workforce development programs.
  • Monitor participant progress toward established goals and modify service plans as needed.
  • Assist participants with crisis intervention, problem-solving, and barrier reduction while maintaining appropriate professional boundaries.
  • Maintain accurate, timely, and confidential participant records, case notes, assessments, referrals, and outcomes in accordance with agency policies and funding requirements.
  • Collect program data and assist with quality improvement initiatives, grant reporting, and performance outcome measures.
  • Participate in multidisciplinary team meetings, case conferences, trainings, and community collaborative meetings.
  • Represent the Center of Excellence at outreach events, networking opportunities, and community presentations.
  • Maintain knowledge of local resources, emerging community needs, and best practices in youth engagement and care coordination.
  • Perform other duties as assigned.
