Key Responsibilities
- Conduct comprehensive admission assessments for new residents and hospital returns, and initiate the development of individualized care plans.
- Support the Nursing Department’s development and operations under the direction of the Director of Nursing (DON).
- Evaluate PRI assessments and coordinate the full scope of the admission process.
- Perform regular audits of resident records to ensure compliance with facility policies and regulatory standards.
- Participate in discharge planning, ensuring smooth transitions for residents.
- Review and update care plans when there is a change in a resident’s condition.
- Complete required documentation, including charting, in compliance with facility policies and procedures.
- Prepare MDS (Minimum Data Set) assessments, care plans, and other required documentation as assigned.
- Uphold confidentiality of all resident information in compliance with HIPAA and company guidelines.
- Supervise, direct, and/or provide resident care as needed.
Requirements
- Active and unrestricted NYS Registered Nurse License.
- Excellent clinical assessment skills.
- Strong independent decision-making abilities.
- Leadership and supervisory skills.
- Proficient in MS Office (Excel, Word, Outlook).
- Long-Term Care Experience Preferred.
- Excellent communication and interpersonal skills.
