About the Role

An Admissions RN provides compassionate care to patients and families beginning their hospice journey. Responsibilities include conducting initial assessments, education, and admissions in various settings, explaining hospice services, and coordinating with interdisciplinary teams for seamless transitions. The role involves emotional support, documentation, and ensuring eligibility criteria are met.

Responsibilities

• Serve as the first clinical contact for new patients and families
• Perform education, assessments, and explain hospice benefits
• Complete documentation and coordinate with team members

Requirements

• Current RN license in Michigan
• At least 2 years of clinical experience, preferably in hospice, palliative, or acute care
• Excellent communication, organizational, and documentation skills
• Valid driver’s license and reliable transportation for local travel
