Responsibilities
- Conduct daily Start of Care (SOC) visits and comprehensive patient assessments in the home
- Develop, implement, and evaluate individualized plans of care in collaboration with physicians and the interdisciplinary care team
- Complete the admission process, including obtaining informed consent, reviewing patient rights, advance directives, emergency preparedness, and agency policies
- Accurately complete OASIS documentation and ensure compliance with Medicare, Medicaid, state, and agency regulations
- Communicate assessment findings, admission orders, care coordination needs, visit frequencies, and identified risks to the Clinical Management team and physicians via Admission report
- Provide skilled nursing care, including medication administration, wound care, treatments, and patient education
- Educate patients and caregivers on disease management, medications, home safety, and the home health plan of care
- Respond to urgent patient needs and clinical situations promptly and professionally
Requirements
- Current, active Registered Nurse (RN) license
- Valid driver’s license and reliable transportation
- At least one year of nursing experience; home health and OASIS experience preferred
- Knowledge of OASIS, Medicare Conditions of Participation, and home health regulatory requirements
- Strong assessment, critical thinking, and time management skills
- Proficient with electronic medical records (EMR); WellSky Kinnser experience is a plus
