As a Centralized Admissions Coordinator, you'll serve as a vital link between hospitals, referral partners, families, and Wellspring's skilled nursing communities.
Working from our Frankenmuth office, you'll help evaluate referrals, coordinate admissions, and guide prospective residents through the transition into care. This position supports all four of Wellspring's skilled nursing facilities throughout Michigan. While you'll have ownership of your work, you won't be doing it alone. You'll be part of a centralized admissions team alongside other dedicated Admissions Coordinators who work together to manage referrals, solve challenges, and support one another every day.
Responsibilities
As a Centralized Admissions Coordinator, you will guide referrals from initial inquiry through successful admission, helping ensure residents receive the right care at the right time. Key responsibilities include:
- Reviewing and responding to referrals promptly while evaluating each opportunity from clinical, financial, and operational perspectives
- Coordinating the admissions process from inquiry through placement and helping create a smooth transition into care
- Completing insurance verifications, pre-certifications, and required documentation while ensuring information is accurate, current, and compliant
- Maintaining real-time referral updates and organized records to support efficient decision-making and admissions workflows
- Partnering closely with Administrators and campus leadership across Wellspring's skilled nursing communities to support occupancy goals, census management, and payer mix objectives
- Building strong relationships with referral partners, residents, and families by providing timely follow-up, responsive communication, and exceptional customer service
- Monitoring referral trends, evaluating conversion outcomes, identifying barriers to admission, and contributing ideas that improve processes and overall admissions performance
Qualifications
The strongest candidates will have:
- Healthcare experience and clinical knowledge, with a Licensed Practical Nurse (LPN) in Michigan preferred
- Three or more years of experience in healthcare admissions, long-term care, sales, marketing, insurance, or a related healthcare role preferred
- Strong clinical and financial assessment skills
- Excellent communication and relationship-building abilities
- Ability to analyze information and make informed decisions
- Strong time management, organization, and prioritization skills
- Ability to collaborate effectively with referral partners, families, and interdisciplinary healthcare teams
Schedule and Compensation
This is a full-time, 40 hours/week position with a schedule of 11:00 a.m. – 7:00 p.m., Monday - Friday, and every third weekend on-call rotation from home. The office is based in Frankenmuth, supporting multiple skilled nursing communities across Wellspring. The annual salary range is $62,400 - $72,800, based on experience and licensure.
