About the role

At Wellspring Skilled Nursing & Rehab, we're looking for a Campus Admissions & Clinical Documentation Coordinator who understands that every admission is more than paperwork—it's the beginning of someone's journey with us. As the first point of connection for many residents and families, you'll play a vital role in creating a smooth, supportive transition into our community. You'll combine your clinical knowledge, communication skills, and attention to detail to ensure residents receive the care they need while helping families navigate an often emotional process. At the same time, you'll help ensure the accuracy of clinical documentation, insurance authorizations, and communication processes that support quality care throughout a resident's stay. Most importantly, you'll help residents and families feel welcomed, informed, and cared for from day one.

Responsibilities

  • Help families move through the admissions process with confidence, answering questions and ensuring they feel supported during an important life transition
  • Work closely with centralized admissions, nursing leadership, therapy teams, and other departments to coordinate admissions, discharges, and bed availability so residents can move into the community smoothly
  • Prepare for each admission by ensuring rooms, equipment, and campus teams are ready to welcome new residents
  • Conduct tours and serve as an ambassador for the community, helping prospective residents and families envision the care, support, and services available to them
  • Use clinical knowledge to support documentation processes, insurance authorizations, Medicare certifications, and communication with MDS teams to ensure accurate and timely reporting
  • Collaborate with interdisciplinary teams to ensure important clinical changes are communicated and documented appropriately throughout a resident's stay

Qualifications

  • Current Michigan Licensed Practical Nurse (LPN) license OR Registered Nurse (RN) license, required
  • Minimum of two years of skilled nursing, rehabilitation, or long-term care experience preferred
  • Strong communication and customer service skills
  • Experience with clinical documentation, insurance processes, admissions, or care coordination is highly valued
  • Ability to work collaboratively with residents, families, nursing teams, and healthcare professionals
  • Strong organizational skills and attention to detail

Pay

Hourly $33.00 - $38.00

Schedule

  • Full-time, Business hours, 8-hour shifts
  • Participation in manager nursing on-call rotation - every 4 weeks
  • Weekday schedule, Monday - Friday
  • On-site
