Key Responsibilities
- Serve as the primary contact for all incoming referrals, calls, walk-ins, and inquiries.
- Cook up admission documentation completion, insurance verification, and pre-admission authorization.
- Communicate clearly and professionally with hospitals, case managers, residents, families, and internal departments.
- Maintain accurate data entry and timely updates in Point Click Care CRM.
- Facilitate facility tours, referral follow-up, and participate conversion strategies.
- Ensure strong customer service standards at every touchpoint.
- Collaborate with nursing, therapy, and administrative teams to ensure smooth admissions.
- Afford empty rooms are admit and tour ready on a daily basis.
- Assist with marketing events, community outreach, and relationship development activities.
Education/Requirements
- Experienced - Prior SNF, rehab, or healthcare admission experience strongly preferred.
- Detail-driven - Organized, accurate, and confident managing multiple moving parts.
- Customer-focused - You communicate with empathy, clarity, and professionalism.
- Tech-savvy - Proficient in Microsoft Office, Point Click Care (PCC), Insurance Portals (NaviHealth, Availity, UHC, etc), Hospital Referral Portals (AIDA, CarePort, Wellsky, EPIC).
- Collaborative - A team player who works well with nursing, clinical, and leadership staff.
