About the role
As an Admission Nurse, you play a vital role in guiding patients and their families through the admission process, ensuring a seamless transition into care. You’ll assess patient needs, complete thorough evaluations, and serve as a compassionate resource for those making critical healthcare decisions.
Responsibilities
- Conduct Patient Assessments - Perform thorough evaluations upon admission, documenting all necessary information in compliance with Medicare regulations.
- Manage Patient & Family Relationships - Provide compassionate guidance, educate families on care options, and ensure admission requirements are met.
- Facilitate Admission Process - Complete necessary paperwork, coordinate with the interdisciplinary team, and ensure a smooth transition for patients.
- Build and Maintain Referral Partnerships - Work closely with referral sources to identify potential patients and promote Vivie’s services.
- Other Duties as Assigned - Report critical patient information to the care team to enhance quality care planning and continuity.
Requirements
- Registered Nurse (RN) license in the state of Minnesota.
- CPR certification is required within six months of hire.
- 1 year experience in hospice, palliative care, or end-of-life care strongly preferred.
- Valid driver's license and use of an insured personal vehicle.
- Ability to pass state mandated background check.
- Physical capability to perform all essential job functions.
- Ability to read, write, and speak English to ensure effective communication with staff, residents, and families.
Schedule
Full-time | 80 hours/pay period | Monday to Friday | 8:00 a.m. to 4:30 p.m.
Pay
Pay Range: $34.50 - $45.00/hour (non-exempt), based on experience, qualifications, and location.
