Pay
Pay Range: $35.34 - $46.00 / hour (non-exempt), based on experience, qualifications, and location.
Territory
Must be available to provide services in our West St. Paul territory
Responsibilities
- Conduct patient assessments - Perform thorough evaluations upon admission, documenting all necessary information in compliance with Medicare regulations.
- Manage patient & family relationships - Provide compassionate guidance, educate families on care options, and ensure admission requirements are met.
- Facilitate admission process - Complete necessary paperwork, coordinate with the interdisciplinary team, and ensure a smooth transition for patients.
- Build and maintain referral partnerships - Work closely with referral sources to identify potential patients and promote Vivie’s services.
- Other duties as assigned - Report critical patient information to the care team to enhance quality care planning and continuity.
Requirements
- Registered Nurse (RN) license in the state of Minnesota.
- CPR certification is required within six months of hire.
- 1 year experience in hospice, palliative care, or end-of-life care strongly preferred.
- Valid driver's license and use of an insured personal vehicle.
- Ability to pass state mandated background check.
- Physical capability to perform all essential job functions.
- Ability to read, write, and speak English to ensure effective communication with staff, residents, and families.
