Admission Nurse (RN) - Hospice

64-80 hours/pay period
Monday to Friday
8 a.m. to 4:30 p.m.

Pay

Pay Range: $35.34 - $46.00 / hour (non-exempt), based on experience, qualifications, and location.

Territory

Must be available to provide services in our West St. Paul territory

Responsibilities

• Conduct patient assessments - Perform thorough evaluations upon admission, documenting all necessary information in compliance with Medicare regulations.
• Manage patient & family relationships - Provide compassionate guidance, educate families on care options, and ensure admission requirements are met.
• Facilitate admission process - Complete necessary paperwork, coordinate with the interdisciplinary team, and ensure a smooth transition for patients.
• Build and maintain referral partnerships - Work closely with referral sources to identify potential patients and promote Vivie’s services.
• Other duties as assigned - Report critical patient information to the care team to enhance quality care planning and continuity.

Requirements

• Registered Nurse (RN) license in the state of Minnesota.
• CPR certification is required within six months of hire.
• 1 year experience in hospice, palliative care, or end-of-life care strongly preferred.
• Valid driver's license and use of an insured personal vehicle.
• Ability to pass state mandated background check.
• Physical capability to perform all essential job functions.
• Ability to read, write, and speak English to ensure effective communication with staff, residents, and families.
