Hospice Admission Nurse (RN)

About the role

We are seeking an experienced Hospice Admissions Registered Nurse (RN) to provide compassionate in-home care to terminally ill patients.

Responsibilities

• Evaluating and admitting patients based on LCD guidelines
• Developing individualized care plans
• Coordinating patient services
• Documenting comprehensive assessments at the point of care
• Participating in staff meetings and on-call rotations

Requirements

• Must accept travel and working conditions as assigned

Qualifications and Skills

• Current RN licensure within the state
• At least one year of clinical nursing experience
• Strong communication, organization, and time management skills
• Ability to work independently and provide effective home healthcare
• Proficiency with computer applications (Windows, Word, Excel)
• CPR certification and a valid driver’s license
• Minimum of twelve hours of continuing education annually
