About the role
Provides direct nursing care to patients with temporary, progressive or permanent physical disabilities, trauma and/or chronic illness. Works with a multidisciplinary team to assess, create, and implement individualized plans of care that help patients adapt to altered lifestyles and achieve maximum function, while supporting patients and their families.
Responsibilities
- Create and implement a written plan of care addressing each patient’s specific needs and goals; make referrals to other professional services as required.
- Modify the plan of care based on reassessment, patient response, progress toward goals, and delegate tasks to team members within their scope of practice.
- Observe, document, and implement planned interventions; communicate the plan through accurate and timely electronic medical record documentation and verbal reports.
- Perform procedures including IV starts/restarts, blood administration, phlebotomy, point-of-care testing under CLIA1988, and respond to call lights.
- Administer medications accurately and timely according to physician orders and organizational policy; monitor for side effects and allergic reactions.
- Transition patients effectively by participating in walking rounds and providing accurate, timely reports to incoming staff.
- Advise physicians and other disciplines, as well as family/significant others, of changes in patient condition or abnormal findings using clear communication.
- Provide patient and/or family education on discharge planning and self-care.
- May precept new employees and oversee other members of the care team.
Education Requirements
- Minimum: Graduate from an accredited school of nursing by date of hire.
- Preferred: BSN.
Experience Requirements
- Minimum: None.
- Preferred: One year as a licensed nurse in a hospital setting.
License/Certification Requirements
- Nursing license in the state of Indiana by date of hire.
- Advanced Cardiac Life Support within 6 months of hire (American Heart Association).
- Basic Life Support (American Heart Association).
- Preferred: Certification in Rehabilitation Nursing (CRRN) or Quality Improvement credential.

3, 12-hour night shifts, 5:00 PM to 5:30 AM.
