About the role
Provides direct nursing care to patients with temporary, progressive or permanent physical disabilities, trauma and/or chronic illness. Works with a multidisciplinary team to assess, create, and implement individualized plans of care that help patients adapt to altered lifestyles and attain maximum function while supporting both patients and their families.
Responsibilities
- Create and implement a written plan of care addressing each patient’s specific needs and goals; make referrals to other professional services as required.
- Modify the plan of care based on reassessment, patient response to treatment, and progress toward goal completion; delegate tasks to team members within their scope of practice.
- Observe, document, and implement planned interventions; communicate the plan through accurate and timely electronic medical record documentation and verbal reports.
- Perform procedures including IV starts/restarts, blood administration, phlebotomy, point-of-care testing under CLIA 1988, and medication administration according to physician orders and policy.
- Maintain knowledge of medication actions and side effects; recognize allergic reactions and document patient responses.
- Transition patients effectively through walking rounds and accurate, timely report to incoming staff.
- Advise physicians, other disciplines, and family/significant others of changes in patient condition or abnormal findings using clear, concise communication.
- Provide patient and/or family education on discharge planning and specific care needs.
- May precept new employees and oversee other members of the care team.
Education Requirements
- Minimum: Graduate from an accredited school of nursing by date of hire.
- Preferred: BSN.
Experience Requirements
- Minimum: None.
- Preferred: One year as a licensed nurse in a hospital setting.
License/Certification Requirements
- Nursing license in the state of Indiana by date of hire.
- Advanced Cardiac Life Support (ACLS) within 6 months of hire (American Heart Association).
- Basic Life Support (BLS) (American Heart Association).
- Preferred: Certification in Rehabilitation Nursing (CRRN) or Quality Improvement credential.
Licenses & Certifications
- Required: Registered Nurse.
- Required: BLS – Basic Life Support.
- Preferred: ACLS.
Schedule
- 2, 12-hour night shifts per week, 5:00 PM – 5:30 AM.
