About the role

Responsible for assisting with planning, coordinating, and implementing activities for the Adult Day Health Center in collaboration with the Activities Coordinator and Lead CNAs. Reports directly to the Clinical Services Director.

Responsibilities

  • Collaborates with the Activities Coordinator, lead CNAs, and Recreation Therapist (RT) to develop and implement specific programming as part of the plan of care for participants, including recreational and social activities.
  • Documents participant participation in the medical record.
  • Plans and implements both group and individual activities based on participant interest in collaboration with the Activities Coordinator, RT, and Center Lead CNAs.
  • Communicates any changes in participant condition to IDT team members.
  • Builds a sense of community and a warm social atmosphere within the Center, helping participants identify a support system.
  • Helps plan a monthly/daily calendar for the Adult Day Health Center based on participant interest and in collaboration with the Activities Coordinator and Center Lead CNAs.
  • Supports Senior TLC’s mission, vision, and values in daily work.
  • May collaborate with the marketing team and other disciplines to develop the Senior Total Life Care quarterly newsletter.
  • Assists with monthly caregiver support group, Participant Advisory Council (PAC), and other specialized group activities as planned.
  • Participates in Quality activities as appropriate.
  • Coordinates with various community resources.
  • If CNA certified or appropriately trained, may help with center/home personal care services, including on-call duties during weekdays/weekends and providing personalized care in the home.
  • Performs other duties as assigned.

Requirements

  • High School Graduate or GED (minimum).
  • Minimum 1 year of experience working with the elderly in a social or recreational program.
  • Experience providing or assisting with coordinating services for a frail elderly population within the past 5 years preferred.
  • NC Certified Nursing Aide (CNA), National and/or NC Activity Director, or NC Licensed Recreational Therapy Assistant (LRTA) certifications preferred but not required.
  • Must be able to work effectively in a team environment.
  • Good understanding of psychosocial problems of aging.
  • Must meet a standardized set of competencies for the position established by Senior TLC and approved by CMS before working independently.
  • Ability to speak fluent Spanish preferred but not required.
  • Must be medically cleared for communicable diseases and have all immunizations up to date before engaging in direct participant contact.
