Active Primary Care APP (NP/PA)- Telehealth
About the Role
For our client, we are seeking an Active Primary Care APP (NP/PA) - Telehealth to join the team of a leader in the Hospitals & Medical Centers space. This role will support clinical priorities focused on quality, patient-centered outcomes, and effective care delivery. You will collaborate with clinical leaders, operational teams, and cross-functional stakeholders to improve standards, workflows, and execution. The position offers the opportunity to strengthen care quality, team performance, and measurable outcomes within a healthcare environment.
Location
Remote - US based candidates only, no visa sponsorship available
Compensation
$125,000 annually
Responsibilities
- Provide virtual primary care via telehealth, including assessments and interventions
- Diagnose and manage chronic conditions, developing individualized care plans
- Evaluate and treat acute concerns through virtual care li>Order and interpret lab tests and coordinate follow-up care
- Manage medication prescriptions, ensuring adherence and patient safety
- Offer health coaching and education to empower patients
- Identify patients needing referrals and facilitate coordination with specialists
Qualifications
- Active Nurse Practitioner (APRN) license in good standing, able to obtain multi-state licensure within 90 days of hire
- Board certification in Family Medicine, Adult-Gerontology, or Internal Medicine
- Minimum 2 years of primary care, urgent care, or internal medicine experience, preferably in telehealth
- Active BLS certification at the time of hire
- Flexibility to work varying shifts and possible weekends to meet patient care needs
