ACT Team Specialist

About the Role

As a member of an Assertive Community Treatment (ACT) team, you will function as a case manager with primary responsibility for providing assertive treatment services, implementing plans for treatment, rehabilitation, and support services. You will coordinate, monitor, and provide direct services to members in a community setting. Your role includes participating in the development of individualized service plans (ISPs) and providing or coordinating services related to these plans. You will act as a member advocate, coordinate services and discharge plans, and assist in crisis intervention. Additionally, you will provide after-hours care, on-call coverage, and support to all members served by the ACT team.

All team members are expected to provide services in accordance with the Nine Guiding Principles for Recovery Oriented Adult Behavioral Health Services and Systems.

Responsibilities

• Provide assertive treatment services and implement treatment, rehabilitation, and support plans
• Coordinate, monitor, and deliver direct services to members in a community setting
• Participate in the development of individualized service plans (ISPs)
• Provide and coordinate services related to ISPs
• Act as a member advocate and coordinate services and discharge plans
• Assist in crisis intervention
• Provide after-hours care, on-call coverage, and support to all members served by the ACT team

Requirements

• Master's or bachelor's degree in a behavioral health or related field
• Bachelor's degree in a non-related field PLUS one year of related experience
• Associate's degree in any field PLUS one year of related experience
• High school diploma or equivalency certificate PLUS three years of related experience
• Minimum one year of experience working with seriously mentally ill adults; previous experience working on an ACT team or equivalent preferred
• Maintain current CPR, First Aid, and Therapeutic Options certifications (offered through Southwest Network)
• Maintain a valid Arizona driver's license and car insurance
• Must be at least 21 years of age

Skills

• Ability to maintain excellent member and family relationships and provide excellent customer service
• Effective written, verbal, and interpersonal communication skills
• Strong organizational, critical thinking, time management, and problem-solving skills
• Ability to utilize multiple types of office equipment (e.g., phones, copier, fax)
• Typing speed of 25 to 40 WPM preferred
• Proficiency in Microsoft Office software
• Ability to utilize the internet and various web browsers

Our Assertive Community Treatment (ACT) program focuses on providing intense, frequent, face-to-face services to members with persistent and severe mental illness and/or substance use disorders. Its primary goal is recovery through community treatment and habilitation. Most services are provided in the community, when and where members need them. ACT services reduce crises and help members learn skills needed to integrate into the community and live independently without supports. This evidence-based practice helps improve outcomes for people who are most at-risk of homelessness, psychiatric hospitalization, and institutional recidivism.
