Full-time position (1.0 FTE) within the Assertive Community Treatment (ACT) department, Behavioral Health Services.
About the Role
Provide comprehensive, community-based mental-health services to adults living with serious and persistent mental illness. As part of a multidisciplinary team, the Case Manager delivers individualized, recovery-oriented care designed to promote stability, independence, and improved quality of life.
Schedule
Monday – Friday, 8:00 AM – 4:30 PM, with occasional evenings and weekends.
Responsibilities
- Conduct client assessments.
- Develop and implement person-centered treatment plans.
- Coordinate medical and behavioral-health services.
- Provide crisis intervention, including rotating on-call shifts for evenings/weekends.
- Obtain medication-management certificate within the first 3 months of employment.
- Assist with medication support.
- Connect clients to community resources and advocate for client needs.
- Deliver services primarily in the community—clients’ homes, shelters, and other non-office settings.
- Transport clients in agency vehicles and, occasionally, personal vehicles.
- Complete all documentation within established guidelines and prioritize client needs in written records.
- Demonstrate understanding of funder and accreditation standards.
- Track and document monthly outcome objectives.
Requirements
- Bachelor’s degree in Human Services or related field.
- One year of mental-health experience.
- Valid driver’s license (required when driving any vehicle for work-related reasons).
- Must obtain medication-management certification within 3 months of hire.
- CPR/First Aid training.
Skills
- Fluent in spoken and written English.
- Proficient in mental-health terminology.
- Experience documenting in an electronic health record (EHR) and using Microsoft Office.
- Ability to present information and respond to questions from clients, medical professionals, management, staff, and the public.
