Youth Outreach Case Worker

About the role

Outreach Case Workers in our Community Connection (PCC) program work with youth to focus on their welfare, helping them avoid negative impacts and succeed in their futures. Responsibilities include identifying goals, tracking progress, coordinating needed services, providing warm referrals, individualized support and advocacy, and facilitating groups. Positions are available in both our Pittsfield and North Adams offices.

Responsibilities

• Build trusting relationships while maintaining appropriate boundaries.
• Coordinate with counseling and mentoring personnel to facilitate multi-systemic wraparound services for at-risk youth (ages 10-24) within Pittsfield, MA.
• Identify youth goals, strengths, and track progress.
• Coordinate needed services and provide warm referrals.
• Offer individualized support and advocacy.
• Facilitate groups for youth.
• Build community connections and work as part of a supportive team.

Requirements

• Ability to meet individuals where they are, both emotionally and physically.
• Knowledge and understanding of systemic, socio-economic, educational, and other challenges faced by youth.
• Informed trauma care and knowledge of gang culture.
• Some evening and weekend work required.
• Strong computer and internet skills, including Microsoft Office.
• Data collection experience is a plus.

Qualifications

• 4 years of relevant experience or a Bachelor’s degree in a human service field preferred.
• Outreach and case management experience strongly preferred.
• Lived experience, multilingual, or multicultural fluency are a plus.

Pay

$22.35 hourly wage.
