About the role
The Prevention & Outreach Specialist will support youth and young adults who are homeless or experiencing unstable housing by connecting them to essential services and community resources.
Responsibilities
- Connect youth to food assistance programs (SNAP, food banks, meal services)
- Support applications for rent, utilities, and relocation assistance
- Guide youth through financial aid, housing, healthcare, and education resources
- Develop individualized action plans for stability and growth
- Build relationships with community providers and service organizations
- Advocate for youth and help remove barriers to accessing services
- Connect youth to job training, internships, and employment opportunities
- Maintain accurate, confidential records and track program outcomes
- Collaborate with team members and participate in case discussions
Requirements
- High school diploma or equivalent required; degree in Social Work, Human Services, or related field preferred
- Valid Driver’s License
- Experience working with at-risk youth or in a community services setting is highly desirable
- Knowledge of local and national systems and resources available for youth, including food assistance, housing, mental health services, and employment programs
- Strong communication and interpersonal skills with an ability to build trust and rapport with youth from diverse backgrounds
- Demonstrated ability to maintain confidentiality and handle sensitive information appropriately
- Able to work independently and as part of a multidisciplinary team
- Proficiency in using computers and basic office software (e.g., Microsoft Office, Google Workspace)
Qualifications
- High school diploma or equivalent required
- Preferred degree in Social Work, Human Services, or related field
- Valid Driver’s License
