Wraparound Facilitator (Case Manager I, II or III)

About the role

The role involves providing wraparound services to individuals and families in need, focusing on case management at various levels.

Responsibilities

• Assess client needs and develop comprehensive care plans
• Coordinate services across multiple agencies and community resources
• Maintain regular contact with clients to monitor progress and adjust plans as necessary
• Provide crisis intervention and support as needed
• Document all interactions and activities in accordance with agency policies and procedures

Requirements

• Bachelor’s degree in social work, counseling, psychology, or related field
• At least 2 years of relevant experience in case management or related field
• Licenses/certifications in case management or related fields preferred
• Strong interpersonal and communication skills
• Ability to work independently and as part of a team
• Proficient in Microsoft Office Suite

$25.00 - $35.00 per hour, depending on experience and qualifications.

Schedule
