Responsibilities

  • Coordinate and administer physician-ordered wound care and treatments
  • Perform skin assessments on all new admissions and ongoing residents
  • Monitor, photograph, and document wound progress and interventions
  • Communicate changes in wound condition to physicians, nursing leadership, and families as appropriate
  • Maintain accurate and timely wound documentation and treatment records
  • Participate in care planning, MDS support, and interdisciplinary collaboration related to wound care
  • Auxiliary with tube feedings and related treatments as assigned
  • Order wound care supplies and maintain appropriate inventory levels
  • Participate in quality assurance, survey readiness, and audits related to skin integrity
  • Provide education and guidance to nursing staff on wound care practices
  • Complete incident reports and documentation related to skin or treatment concerns
  • Ensure treatment areas are clean, safe, and compliant with infection control standards

Qualifications

  • Current, active RN or LPN license in the state
  • Wound care or treatment nursing experience preferred
  • Strong knowledge of skin integrity, wound staging, and treatment protocols
  • Excellent documentation and organizational skills
  • Ability to work independently and meet regulatory timelines
  • Strong communication and interdisciplinary collaboration skills

Physical Requirements

  • Standing and walking throughout the facility
  • Bending, lifting, and assisting residents with positioning and treatments
  • Exposure to clinical environments, bodily fluids, and wound care supplies
