Wound Nurse RN
Brookhaven Health Care Facility is looking to add a Wound Care RN to its clinical team. In this role, you'll provide specialized wound care services while supporting resident healing, infection prevention, and overall quality outcomes. Located in East Patchogue, NY, Brookhaven Health Care Facility is part of The McGuire Group, a recognized leader in skilled nursing, rehabilitation, and long-term care services throughout New York State.

Responsibilities
- Provide direct nursing care to residents in accordance with care plans, physician orders, and wound care protocols
- Assess, monitor, and document wound status, treatment effectiveness, and resident progress
- Administer medications, IV therapies, treatments, and wound care interventions as prescribed
- Collaborate with physicians, nursing staff, therapy teams, and other interdisciplinary team members to support resident recovery and care goals
- Develop, implement, and update wound care plans as needed
- Monitor for signs of infection, skin breakdown, and changes in resident condition, reporting concerns promptly
- Supervise and support CNA and LPN staff to ensure quality resident care
- Maintain accurate documentation, assessments, care plans, and clinical records
- Follow infection control, resident safety, and HIPAA guidelines
- Perform additional duties as assigned
Requirements
- Valid New York State Registered Nurse (RN) License required
- CPR Certification required or obtained upon hire
- Experience in skilled nursing, rehabilitation, long-term care, acute care, or wound care preferred
- Strong clinical assessment, wound management, and critical thinking skills
- Ability to supervise, delegate, and collaborate effectively with interdisciplinary teams
- Excellent communication, organizational, and documentation skills
- Commitment to providing compassionate, resident-centered care
Pay
Salaried: $104,000 (Hourly Equivalent: $50/hour)
Schedule
Full-Time, Monday–Friday, 8 am – 4 pm
