What You'll Do
- Perform comprehensive skin and wound assessments for residents.
- Develop, implement, and update individualized wound treatment plans.
- Monitor wound healing progress and adjust interventions as appropriate.
- Collaborate with physicians, therapy staff, dietitians, and the interdisciplinary care team.
- Lead and support the facility's skin integrity and pressure injury prevention program.
- Ensure accurate, timely, and compliant clinical documentation.
- Provide education and guidance to nursing staff on wound care best practices.
- Participate in Quality Assurance and Performance Improvement (QAPI) initiatives.
- Monitor infection prevention practices related to wound management.
- Serve as a clinical resource for staff, residents, and families.
- Maintain compliance with all federal, state, and facility regulations.
- Perform other nursing duties as assigned.
Qualifications
- Current, active Registered Nurse (RN) license in the State of Oregon.
- Strong assessment, documentation, and clinical decision-making skills.
- Knowledge of infection prevention and skin integrity management.
- Excellent communication and interpersonal skills.
- Ability to work collaboratively with residents, families, physicians, and interdisciplinary teams.
- Passion for providing exceptional care to seniors in a skilled nursing environment.
Preferred
- Previous wound care experience in a skilled nursing, rehabilitation, long-term care, or post-acute setting.
- Wound Care Certification (WCC, WOCN, or equivalent) preferred—or willingness to obtain certification with employer support.
- Experience with pressure injury prevention and treatment protocols.
- Experience in electronic medical records (EMR) documentation.
