About the role
The Wound Care Nurse is responsible for assessing, treating, documenting, and monitoring residents with wounds, pressure injuries, and skin integrity concerns. This position collaborates closely with nursing leadership, physicians, and interdisciplinary teams to ensure best practices and positive outcomes.
Responsibilities
- Perform comprehensive skin and wound assessments
- Obtain and carry out wound care treatment orders
- Monitor wound healing progress and recommend treatment changes
- Document assessments, treatments, and outcomes accurately
- Track pressure ulcers and skin alterations, including weekly measurements and reports
- Implement and monitor preventive interventions for at-risk residents
- Coordinate wound care supplies with central supply
- Absorb and educate nursing staff on wound care protocols and best practices
- Participate in Quality Assurance and Performance Improvement (QAPI) initiatives
- Stay current on state, federal, and industry wound care standards
Requirements
- Graduate of an accredited RN or LPN program
- Active nursing license in the state of Missouri (required)
- Minimum 1 year wound care experience in a long-term care setting preferred
- 2+ years of skilled nursing facility experience preferred
- Current BLS/CPR certification
- Strong assessment, documentation, and communication skills
- Knowledge of skin integrity programs, prevention strategies, and regulatory requirements
