Wound Care Nurse RN or LPN

Key Responsibilities

• Evaluate wounds to determine their type, stage, and size, and then create and implement individualized treatment plans.
• Perform wound care duties such as cleaning, debriding (removing dead tissue), and applying dressings, while ensuring sterile techniques are used.
• Monitor patient progress and document all care provided, including for reimbursement purposes like Medicare.
• Educate residents, their families, and other staff on wound care, infection prevention, and injury prevention techniques, particularly focusing on preventing pressure ulcers.
• Collaborate with a multidisciplinary team, including physicians, to coordinate care and make necessary adjustments to treatment plans.
• Write nursing orders that support wound healing and help prevent skin breakdown.
• Monitor for signs of infection and implement protocols to prevent it.
• Stay current with the latest wound care research, techniques, and products.
