About the role
Provide routine daily nursing care and services in accordance with each resident’s assessments and care plan, specializing in the care of patients with wounds, ostomies, or continence disorders.
Responsibilities
- Works with patients who have gastrointestinal stomas for feeding tubes or colostomy bags, skin ulcers, drains, surgical incisions, or pressure sores.
- Evaluates and provides treatment protocols to prevent skin irritation and infection for wounds that may not heal well due to conditions such as diabetes or poor circulation.
- Performs administrative duties such as completing medical forms, reports, evaluations, studies, and charting.
- Implements standards of nursing practice in infection prevention, reviewing and revising as necessary to reflect changes in nursing practice and resident population.
- Assists with the provision of nursing services 24 hours a day, seven days a week.
- Provides licensed care to assigned residents as ordered by a physician and in accordance with facility, federal, state, and local standards, guidelines, and regulations.
- Charts nurses' notes in an informative and descriptive manner that reflects the care provided and the resident’s response to the care.
- Monitors and directs non-licensed ancillary services to meet professional standards and maintain the highest practicable level of resident care.
- Communicates and reports information to residents, resident families, physicians, guardians, governmental agencies, and facility personnel as required by facility policy and State/Federal guidelines.
- Follows and adheres to company policies and procedures regarding HIPAA compliance.
- All other duties as assigned.
Requirements
- Must be CPR trained and possess a current BLS certification (recognized organizations include the American Heart Association and the American Red Cross).
- Excellent written and verbal communication skills in English.
- Ability to multi-task and remain calm under stress.
- Analytical and problem-solving skills.
- Understands and follows infection control standards, policies, and procedures.
- Must be able to adapt to a flexible work schedule and occasional overnight travel.
Qualifications
- Nursing degree or diploma from an accredited college or university.
- Current license to practice in the state as a Licensed Practical Nurse (LPN).
- 1 year of experience preferred.
- Long-term care experience preferred.
Skills
- Serves as the expert in wound care.
Physical Requirements
- Walking, reaching, bending, lifting, extended sitting, grasping, fine hand coordination, pushing and pulling, with or without the aid of mechanical devices.
- Ability to distinguish smells and temperatures.
- Limited potential for exposure to environmental hazards.
- Understanding and adherence to company safety standards and protocols required.
