About the role

Community Healthcare Network is a Federally Qualified Health Center (FQHC) serving underserved communities in Brooklyn, Bronx, Manhattan, and Queens, touching the lives of 85,000 people annually. We are seeking a Women's Health Nurse Practitioner/Clinician for our Health Centers based in these boroughs. Reporting to the Medical Director, this individual will be responsible for providing evidence-based Primary Care to our patients and is an integral part of the collaborative care team which includes nursing staff and services such as Health Education, Nutrition, Social Work, and Behavioral Health.

Requirements

• Valid N.Y.S. License to deliver medical care
• At least one (1) year of post-graduate/residency experience delivering care in a Primary Care setting
