About the role

Community Healthcare Network, a Federally Qualified Health Center (FQHC), serves underserved communities across Brooklyn, Bronx, Manhattan, and Queens, touching the lives of 85,000 people annually through health centers and various programs. We are seeking a Women's Health Nurse Practitioner/Clinician for our Health Centers based in these boroughs.

Responsibilities

• Providing evidence-based Primary Care to patients
• Serving as an integral member of the collaborative care team, which includes nursing staff and other services such as Health Education, Nutrition, Social Work, and Behavioral Health

Requirements

• Valid N.Y.S. License to deliver medical care
• At least one (1) year of post-graduate/residency experience delivering care in a Primary Care setting
