Responsibilities
- Provides wellness assessment and direct services to people with serious mental illness enrolled in the PACT Program.
- Develops psychosocial and comprehensive assessments and an individualized recovery plan.
- Ensures timely and quality documentation to adhere to DMHAS/Medicaid standards.
- Participates in clinical and non-clinical team meetings, including individual and team supervision.
- Sets up monthly supervision from supervisor.
- Teaches skills to support each person's individualized recovery journey and community integration.
- Assists in securing safe, clean, affordable housing, food, and clothing, and provides linkage to entitlement benefits.
- Observes and assesses service recipients' concerns, mental illness symptoms, and behaviors in response to medication, and monitors for medication side effects.
- Provides medication education for individuals and families/significant others with service recipients' consent.
- Coordinates services with other community mental health and non-mental health providers, as well as other medical professionals.
- Trains and instructs individuals at home and in the community on personal hygiene tasks, household chores, money management, and community transportation.
- Provides wellness services including education and cognitive/behavioral interventions on nutrition, diet, exercise, stress reduction strategies, and other wellness tools.
- Rapidly and flexibly responds to crises, including accompanying individuals to screening centers/psychiatric emergency centers and remaining with them during the assessment process.
- Uses evidence-based practices such as Motivational Interviewing, Cognitive Behavioral Interventions, and Teaching.
- Develops and implements Wellness Recovery Action Plans, instilling hope, promoting choice and empowerment, and incorporating spirituality, in conjunction with the team.
- Supports the service recipient's family and other members of their social network to help manage symptoms and reduce stress.
- Provides education to assist the service recipient and family to relate positively and supportively.
Requirements
- Master’s degree and one-year experience in the provision of MH services, or
- Bachelor’s degree in a behavioral health science from an accredited institution and two years’ experience in the provision of MH services, or
- Associate’s degree in Psychiatric Rehabilitation and two years’ experience in the provision of mental health services.
Valid driver’s license required - No more than one moving violation within the past 12 months. Vehicle required.
