Wellness QA Nurse Manager Wellness

Responsibilities

• Leading quality improvement and corrective action initiatives using HEDIS measures
• Reviewing charts, audits, and population health data to identify trends and gaps in care
• Collaborating with providers and leadership on quality-of-care initiatives
• Managing and supporting Wellness RNs and Health Educators
• Monitoring chronic and behavioral health gaps in care and ensuring follow-through
• Developing strategies to improve clinical performance and patient outcomes
• Supporting patient education initiatives using motivational interviewing techniques

Qualifications

• Active NYS RN License required (BSN preferred)
• 3+ years of Quality Improvement experience required
• 2+ years of leadership or management experience required
• Experience with HEDIS and NCQA standards required
• Background in ambulatory care, community health, or med-surg preferred
• Strong analytical, leadership, and communication skills
• Bilingual Spanish preferred
• Comfortable collaborating across interdisciplinary teams
