Wellness Nurse (LPN- Full Time 3pm-11pm)

Responsibilities

• Provide warm and engaging personal care services, assistance, support, and companionship to residents.
• Administer or assist with medication for residents according to the Resident Care Plan.
• Engage residents while checking on their well‑being and communicate changes in condition or care plan.
• Inspire the team and collaborate with the Wellness Director and Assistant Wellness Director to assess and document resident assessments at state‑specified intervals.

Qualifications

• Positive attitude and love working with people.
• Graduate of a state‑approved (accredited) school of nursing.
• Current state license as a Licensed Practical Nurse (LPN) or Vocational Nurse.
• No experience required – new graduates and experienced nurses are welcome.
• At least 2 years of experience in long‑term care, assisted living, home health, or hospital settings preferred.
• Experience working with residents who have Alzheimer’s disease or related dementias is a plus.
• Ability to handle multiple tasks and follow federal and state regulations and nursing practices.
• Proficiency with computer applications, including Microsoft Office (Windows, Outlook, Excel) and the ability to learn new software.
• Physical ability to stand, walk, sit, lift or carry up to 50 lb, and perform frequent stooping, kneeling, and other body‑mechanics tasks.

Pay

Salary range: $34.00 – $35.00 hourly.

Sign‑on bonus: $2,500 total ($1,250 after 90 days and $1,250 after 180 days).

Schedule

Full‑time evening shift (3:00 pm – 11:00 pm) on a rotating two‑week schedule:

• Week 1: Monday, Tuesday, Wednesday, Thursday, Saturday
• Week 2: Sunday, Tuesday, Wednesday, Thursday, Friday
• Alternate pattern: Week 1 – Sunday, Tuesday, Wednesday, Thursday, Friday; Week 2 – Monday, Tuesday, Wednesday, Friday, Saturday
