About the role

The essential functions of the job for the Licensed Practical Nurse require, and may be amended from time to time at the sole discretion of Viva Senior Living to include, but are not limited to the following:

Responsibilities

• Care coordination
• Supporting residents according to their care plans
• Completing state-required assessments and service plans
• Coordinating services with hospice, home health, and durable medical equipment companies
• Medication management
• Reporting care provided and changes in resident condition
• Responding to resident emergencies
• Documenting incident reports
• Conducting community audits and training
• Participating in resident care activities
• Communicating with residents, visitors, and other personnel
• Arriving on time for work
• Performing other related duties as required

Qualifications

• Valid LPN license in good standing
• High school diploma or GED
• English language proficiency
• Maintain the highest standards in caring for residents
• Adhere to Viva Senior Living’s policies and procedures

Physical and Sensory Requirements

• Ability to move intermittently throughout the workday
• Ability to cope with mental and emotional stress
• Ability to see and hear or use prosthetics if necessary
• Ability to push, pull, move, and/or lift a minimum of 25 pounds to a minimum height of 5 feet and carry such weight a minimum distance of 50 feet
• Ability to assist in the evacuation of residents during emergency situations

Annual Health Requirements

• Negative TB skin test annually
