POSITION SUMMARY
The Wellness Nurse (LPN) plays a vital role in ensuring the health and well-being of our residents. This role collaborates with the Health and Wellness Director to provide high-quality care through health assessments, medication management, and coordination with residents, families, and the care team.
KEY RESPONSIBILITIES
- Perform resident assessments and reassessments according to policies, monitoring any changes in condition
- Communicate with the Health and Wellness Director about resident needs to ensure consistent quality of services
- When necessary, participate in hands-on care, medication administration, and specific nursing tasks
- Schedule and assign caregiving and med tech shifts to ensure adequate coverage and compliance with staffing guidelines and department allowances
- Supervise, train, and support care team members, promoting teamwork and adherence to care standards
- Assist in the transcription of physicians' orders and the review of new physicians' orders
- Maintain accurate and up-to-date resident records, including care plans, incident reports, and medical documentation
- Caregiver planning with home health agencies on-site, working within community and company policies
- Address residents’ special needs, requests, and complaints
- Coordinate admissions with the sales team, including supervising move-ins
- Understand the community's care regulations along with state-specific regulations
- Facilitate communication between residents, families, and healthcare providers to ensure seamless care delivery
CANDIDATE QUALIFICATIONS
- Education and Certifications:
- A valid, incumbent nursing license (LPN) is required
- A high school diploma or equivalent
- An associate degree or higher in nursing is preferred, or a combination of education and experience is required. Additional coursework in management is an asset
- Experience, Competencies, and Skills:
- At least four (4) years working in geriatric nursing, including experience with medication administration, delivery systems, and pharmacies
- At least one (1) year of relevant supervisory experience, including staff scheduling and management, is preferred
- Experience with memory care is a plus
- Excellent attention to detail and high integrity

- Ability and willingness to participate in on-call rotation
- Must meet all mandatory health requirements according to state regulations
