POSITION SUMMARY

The Wellness Nurse (LPN) plays a vital role in ensuring the health and well-being of our residents. This role collaborates with the Health and Wellness Director to provide high-quality care through health assessments, medication management, and coordination with residents, families, and the care team.

KEY RESPONSIBILITIES

  • Perform resident assessments and reassessments according to policies, monitoring any changes in condition
  • Communicate with the Health and Wellness Director about resident needs to ensure consistent quality of services
  • When necessary, participate in hands-on care, medication administration, and specific nursing tasks
  • Schedule and assign caregiving and med tech shifts to ensure adequate coverage and compliance with staffing guidelines and department allowances
  • Supervise, train, and support care team members, promoting teamwork and adherence to care standards
  • Assist in the transcription of physicians' orders and the review of new physicians' orders
  • Maintain accurate and up-to-date resident records, including care plans, incident reports, and medical documentation
  • Caregiver planning with home health agencies on-site, working within community and company policies
  • Address residents’ special needs, requests, and complaints
  • Coordinate admissions with the sales team, including supervising move-ins
  • Understand the community's care regulations along with state-specific regulations
  • Facilitate communication between residents, families, and healthcare providers to ensure seamless care delivery

CANDIDATE QUALIFICATIONS

  • Education and Certifications:
    • A valid, incumbent nursing license (LPN) is required
    • A high school diploma or equivalent
    • An associate degree or higher in nursing is preferred, or a combination of education and experience is required. Additional coursework in management is an asset
  • Experience, Competencies, and Skills:
    • At least four (4) years working in geriatric nursing, including experience with medication administration, delivery systems, and pharmacies
    • At least one (1) year of relevant supervisory experience, including staff scheduling and management, is preferred
    • Experience with memory care is a plus
    • Excellent attention to detail and high integrity
    • Excellent communication skills and teamwork mindset
    • Strong leadership and team management skills
    • Outstanding organizational skills
    • Positive attitude, empathy, patience, and commitment to treating our residents with dignity and respect
    • Ability and willingness to participate in on-call rotation
    • Must meet all mandatory health requirements according to state regulations
